n converting Madge’s current long-acting dose of Oxycontin® 40 mg po q12 hr to short-acting ATC dosing, the appropriate order would be Oxydose® 13 mg q 4hrs ATC with 8-12 mg q2 hr prn for breakthrough pain (24 hr total of long-acting agent divided by 6 for q 4hr dosing according to the medication’s half-life, plus breakthrough pain dosing of 10-15% of 24 hour dosage given q2hr prn per the 2008 American Pain Society guidelines)
